If you know that you would like to be a part of the Children's Ministry, please download, print and submit the following completed forms to the CLFC Children's Ministry Office:

If you are under the age of 18 and volunteering for Children's Ministry, you and your Parent or Guardian must download, print and submit a Teen Approval Form to the CLFC Children's Ministry Office:
